Instructions for filling out NEFP 401(k) participation sheet

Complete the form below. Once completed forward a copy to IBEW investment office by fax, mail, or email.

Contact your employer to inform them of your intention to participate in the NEFP. You will need to provide them a copy of your "participant Information Sheet" which you can download below. Also instruct your employer to contact the office listed above so your enrollment in the plan can be finalized.
